About The Position

This is an excellent position for someone who wants to develop their career in social work or counseling while working with families and schools. This is a new position in the School-Based Early Support Program, a new preventive services model that enhances school-based partnerships. The Community Pathways Worker provides services to 8 families at a time across four different partner schools in Brooklyn School District (15 or 20) focused on case management and referrals to needed resources. The Community Pathways worker will also work with the School Outreach Liaison to implement the Families and Schools Together (FAST) program.

Requirements

  • BA/BS/BSW with relevant experience
  • Bilingual in Spanish required
  • Parent/Caregivers with lived experience navigating resources for their families or special needs children, and/or volunteering in schools encouraged to apply

Nice To Haves

  • Experience working in, or in partnership with, schools preferred
  • Groupwork skills and experience preferred

Responsibilities

  • Support the School Outreach Liaison with school partnership efforts, developing crucial relationships with partner schools and families
  • Responsible for providing case management and counseling services to 8 families across four schools in Brooklyn School District 15 or 20
  • Responsible for completing initial assessments and preparing families for more long-term family counseling support in the Prevention Program.
  • Participate in the implementation of the Families and Schools Together (FAST) program, including co-facilitation of multi-family groups
  • Assist in codesigning and coordinating school-based events
  • Collaborate with CFL program staff, school staff, other community providers, and multiple systems (housing, public assistance) to meet the needs of families
  • Provide direct advocacy and support to families by accompanying them to access services in the community and connecting them with needed resources
  • Conduct office and home visits as needed
  • Complete required documentation and data entry
  • Attend staff meetings, supervision, trainings, and meetings within partner schools
  • Build relationships with community-based service providers and other key community stakeholders
  • Collaborate with all CFL programs to ensure an integrated approach to serving families
