Requirements

  • Bachelor’s degree in human services, social work, or a related field preferred.
  • Experience working with individuals experiencing mental health or substance use challenges is highly desirable.
  • Excellent communication, problem-solving, and interpersonal skills.
  • Ability to work independently in community settings.
  • Valid driver’s license and reliable transportation required.

Responsibilities

  • Provide direct support and guidance to individuals in the community who are experiencing mental health or substance use challenges.
  • Meet clients where they are—at home, in the community, or wherever support is needed.
  • Develop and maintain strong, trusting relationships with clients.
  • Collaborate with interdisciplinary teams to create person-centered care plans.
  • Document interactions, progress, and outcomes in accordance with agency policies and regulations.
  • Advocate for clients to ensure access to resources and services.
