Requirements

Bachelor’s degree in human services, social work, or a related field preferred.
Experience working with individuals experiencing mental health or substance use challenges is highly desirable.
Excellent communication, problem-solving, and interpersonal skills.
Ability to work independently in community settings.
Valid driver’s license and reliable transportation required.

Responsibilities

Provide direct support and guidance to individuals in the community who are experiencing mental health or substance use challenges.
Meet clients where they are—at home, in the community, or wherever support is needed.
Develop and maintain strong, trusting relationships with clients.
Collaborate with interdisciplinary teams to create person-centered care plans.
Document interactions, progress, and outcomes in accordance with agency policies and regulations.
Advocate for clients to ensure access to resources and services.
