About The Position

The Community Wellness Manager serves as a vital liaison between families and community resources, developing and maintaining a comprehensive referral system that connects students and families with essential services across surrounding counties. This role combines case management, data tracking, and collaborative partnership-building to ensure families receive timely access to needed supports beyond the school environment.

Requirements

  • High school diploma required; Associate's or Bachelor's degree preferred
  • Knowledge of community resources available to families within surrounding counties and throughout the state
  • Strong case management and follow-up skills
  • Proficiency with technology systems for data management, communication, and needs assessment
  • Excellent interpersonal and communication skills with ability to engage diverse populations
  • Demonstrated ability to maintain accurate records and produce data reports
  • Professional demeanor with strong organizational and time management skills
  • Proactive, solution-oriented work ethic
  • Ability to work independently and collaboratively as part of a team

Responsibilities

  • Develop, maintain, and expand a comprehensive system connecting families with local community resources and student support services across surrounding counties
  • Manage case referrals from Licensed Clinical Social Workers (LCSWs), providing resource coordination, assistance, and ongoing follow-up for families requiring supports beyond school-based services
  • Utilize technology platforms to assess family needs, coordinate services, and maintain detailed quantitative and qualitative data on service delivery and outcomes
  • Implement multiple outreach strategies to increase the number of families successfully connected to appropriate community resources
  • Collect and analyze feedback from families to continuously improve service delivery and system effectiveness
  • Provide regular reports and data analysis regarding system effectiveness, along with recommendations for program improvement
  • Collaborate with professional colleagues to enhance service delivery and expand professional knowledge
  • Maintain professional communication with diverse stakeholders in a personable and engaging manner
  • Proactively identify barriers to service access and develop solutions to address emerging family needs
  • Perform additional duties as assigned to support community wellness initiatives
