About The Position

Under general supervision of the Program Manager – Intake & Assessment is responsible for the implementation and coordination of consumers in the Early Start Program intake process. Completes phone referrals, as well as reviews referrals and documents from social work agencies, hospitals, and health care offices. Responsible for inquiries and assignments related to closed and inactive cases. Requires good work habits including regular attendance and punctuality. Has no supervisory responsibility.

Requirements

  • High school diploma
  • Two (2) years of office and administrative experience.
  • Ability to handle change well and be flexible and adaptable in dealing with interruptions, new priorities, and new assignments.
  • Ability to follow oral and written direction.
  • Ability to establish and maintain effective working relationships with others.
  • Good verbal and written communication skills.

Nice To Haves

  • Bilingual preferred.

Responsibilities

  • Perform daily activities to facilitate the intake process for the agency’s Early Start Program.
  • Promptly and courteously answer incoming referral calls for the program.
  • Transfer calls to requested office or person.
  • Discern whether initial intake phone call requires additional follow-up and make the necessary contacts.
  • Fill out appropriate referral information paperwork completely and accurately.
  • Daily, obtain case assignments from Program Manager, and complete all paperwork and the necessary IT processing for all cases opened to the Early Start caseloads.
  • Distribute and route the consumer record and medical reports to appropriate staff on a timely basis.
  • Return phone calls promptly regarding eligibility.
  • Handle all phone calls and mail and is responsible for administrative work on inactive or closed cases.
  • Defer program denial questions and/or calls to Program Manager.
  • Process cases and route referral paperwork to Case Control for immediate processing.
  • Track Department of Children Services (DCS) cases and follow-up to obtain educational rights if needed.
  • Coordinate with Local Education Agencies (LEA) regarding Solely Low Incidence cases and capacity openings for referrals.
  • Delegate tasks and route incoming phone calls regarding referrals from foster agencies to Early Start CST II/Consumer Intake Clerk.
  • Assist in the collection of information, prepare reports and perform surveys related to the intake process as requested and on a timely basis.
  • Keep log of eligible and ineligible referrals for statistical and reporting purposes.
  • As requested by the Program Manager of Early Start, assist in tracking and assigning transfer-in cases on a timely basis.
  • Coordinate with vendored programs to refer cases for developmental assessments.
  • Assist in locating record for pending mediation and/or appeal.
  • On a daily basis, maintain an adequate skill level in interpersonal and community relationships.
  • Communicate effectively with the families and consumers utilizing the agency’s intake services.
  • Take or relay information related to the Early Start Program.
  • Give public and employees general procedural information related to Early Start Intake and any necessary forms.
  • Process faxes via e-mails, screen and route to appropriate consumer records.
  • Work cooperatively and effectively with others.
  • Perform as a member of the team, answer questions, share expertise and contribute to the harmony of the team.
  • Use office equipment appropriately and report the need for any repairs.
  • Keep work area neat and orderly.
  • Observe all safety rules and comply with IRC’s Injury and Illness Prevention Plan and Workplace Violence Prevention Plan.
  • Ability to handle a hybrid work environment consisting of working in the office and remotely at home while maintaining work productivity and efficiency; communicate via telephone calls, video conference calls, emails, and chat rooms.

  • Utilize agency’s IT systems as assigned, maintaining security and following agency’s protocol and procedures, rules, and requirements.
  • Actively work to maintain and expand knowledge base.
  • Attend training sessions as required.
  • Perform different or additional work as assigned.
