The Community Coordinator provides instruction and supervision to people participating in the Community Services Programs, including Community Habilitation and In Home Respite. The Community Coordinator works with individuals in their own homes and in the community to increase their independence in activities of daily living. The Community Coordinator may provide an assessment of new program participants. Provide training and assists with orientating newly hired staff. Assists the Senior Community Supervisor with monthly summaries, reviewing billing, writing Staff Action Plans and attending Life Plan meetings on behalf of the Senior Community Supervisor. The Community Coordinator may also assist the Senior Community Supervisor with covering cancellations/staff PTO as needed.
