Job Summary: Contributes to excellence in patient/family centered care by coordinating the health care of select patient populations/designated services across the continuum of care using the nursing process. Promotes the safety and well being of all patients by coordinating the patient's discharge from the hospital and transition of care to the out-patient clinic. Collaborates with the inpatient interdisciplinary team to develop and maintain the plan of care. Primary duties and responsibilities: Discharge and Transition Management: 1. Promotes the safety and well being of all patients by coordinating the patient's discharge from the hospital and transition of care to the out-patient clinic. 2. Collaborates with the inpatient interdisciplinary team to develop and maintain the plan of care. 3. Works in collaboration with others to ensure that all discharge needs are met. 4. Schedules all discharge appointments, treatments, and protocol specific orders. 5. Completes needed discharge follow up phone calls to patients and documents appropriately. Professional Development: 1. Works with manager to formulate plan for professional development. 2. Attends educational in-services as appropriate. 3. Participates in professional activities and organizations to maintain knowledge of current trends, practices, and developments. Performs other duties as required.

Requirements

  • Bachelors degree in Nursing.
  • Three years of clinical nursing experience as a Registered Nurse with at least two years of recent experience (within the last five years) in the area of concentration.
  • Must have a valid, active unencumbered Nursing license or temporary permit approved by the Georgia Licensing Board.
  • BLS
  • If completing virtual care activities that may include multi-state practice, an active compact/multistate license (eNLC) is required within 60 days of hire. Employees in role prior to 11/09/2025 will have until their next renewal date to achieve compact status.

Nice To Haves

  • OCN certification preferred for some positions.
  • ACLS Certification preferred but may be required in some areas.
