Trillium Health Resources has a career opening for a TCL Quality Management Coordinator to join our team! The TCL Quality Management Coordinator works within the Quality Management department and is responsible for various quality assurance and quality improvement job activities throughout the organization with a specific focus on Transitions to Community Living (TCL) activities. This position reports to the Head of Performance Improvement. This position is responsible for maintaining a working knowledge of applicable Federal, State, and Accreditation requirements and any other document or resources related TCL requirements such as the TCL settlement agreement. This position assists with ensuring the organization maintains compliance with these requirements and those specific to TCL.

Requirements

  • Bachelor’s degree and a minimum of two (2) years' experience in Quality Assurance related to performance improvement, quality management, key quality metrics, root cause analysis, continuous quality improvement, quality improvement, compliance, or data analysis.
  • Must have a valid driver’s license.
  • Must reside within North Carolina.
  • Must be able to travel within catchment as required.

Nice To Haves

  • Experience in a behavioral or managed care environment.
  • Experience in a QA/QI related position and previous participation.
  • Experience with Transitions to Community Living (TCL).

Responsibilities

  • Maintain a working knowledge of all Federal Medicaid regulations, State rules and accreditation standards, TCL requirements including the settlement agreement and Trillium contractual requirements through ongoing review of documents such as contracts and manuals.
  • Serve as the organization’s TCL QAPI point of contact for DHHS. Manage and coordinate the development and implementation of TCL related internal QAPI activities and implementation of State QAPI initiatives.
  • Develop, implement, and evaluate QAPI processes and procedures to monitor and ensure the quality and timelines of contracted TCL functions.
  • Work with colleagues across functional areas to ensure that process and procedural changes are reflected in organizational documents such as policies and procedures.
  • Assist with the development and implementation of Performance Improvement Projects (PIPs) and other performance improvement efforts/QAPI processes to identify and address quality and performance issues.
  • Coordinate and facilitate Root Cause Analyses (RCA’s) process. Work collaboratively with Trillium staff and providers to complete the RCA process, when needed, to determine the root cause(s) of a problem and to develop strategies for correction and prevention of the problem from occurring again in the future.
