About The Position

The Licensed Counselor Intake Services is a core member of Diversus Health’s clinical team and is responsible for delivering high-quality, transparent, and trauma-informed diagnostic intake services for children, adults, and individuals with substance use concerns. This position works full time as an intake therapist and does not carry a caseload. Instead, the counselor completes comprehensive diagnostic assessments, determines appropriate levels of care, develops treatment and safety plans, and coordinates referrals to internal and external services. The Licensed Counselor collaborates with crisis services, schools, first responders, hospital emergency departments, and community partners to support timely access to care. This role is essential in helping clients begin their treatment journey and ensuring they are connected with the most appropriate clinical services.

Requirements

  • Licensed, in good standing, in the State of Colorado as an LCSW, LPC, LMFT, PsyD, or PhD (required).
  • Experience providing therapeutic services to a broad range of populations, from children through adults.
  • Experience using diverse assessment modalities to complete diagnostic intakes and treatment recommendations.
  • Experience working within a team environment.
  • Strong verbal and written communication skills for highquality clinical documentation and client interaction.
  • Ability to perform active listening, ethical decisionmaking, complex problem solving, and clinical judgment.
  • Proficiency with Electronic Health Record (EHR) systems and basic computer applications; ability to type 25+ wpm.

Nice To Haves

  • LAC or SUDrelated experience preferred.
  • Experience in SUD settings or with cooccurring disorders preferred.

Responsibilities

  • Clinical Assessment & Diagnostic Evaluation Conduct comprehensive biopsychosocial assessments addressing psychiatric, emotional, social, developmental, and functional needs.
  • Complete diagnostic evaluations for Child & Family, Adult Outpatient, and Substance Use Disorder (SUD) services.
  • Formulate accurate DSMbased diagnoses using a variety of assessment methods and clinical interview techniques.
  • Develop individualized treatment and safety plans in alignment with Diversus Health standards.
  • Direct Client Services & Intake Functions Deliver intake assessments in a professional, timely, and personcentered manner.
  • Determine the appropriate level of care and make internal referrals accordingly.
  • Facilitate admission to appropriate inpatient or higherlevel services when clinically indicated.
  • Maintain active engagement with crisis hotlines, schools, first responders, community partners, and hospital EDs to support client access and continuity of care.
  • Crisis Response & Risk Management Assess and respond to risk related to suicidality, selfharm, psychosis, substance use, and imminent safety concerns.
  • Develop client-specific safety plans and risk mitigation strategies.
  • Coordinate with crisis teams and emergency services to ensure safe and appropriate crisis intervention.
  • Care Coordination & Collaboration Collaborate with internal clinical teams, community resources, and external partners to ensure seamless client transitions postintake.
  • Maintain open communication with referring providers, school personnel, emergency departments, and appropriate community agencies.
  • Support clients in accessing needed services, including outpatient therapy, SUD treatment, psychiatric care, and community supports.
  • Documentation & Compliance Complete accurate, timely documentation in the Electronic Health Record (EHR).
  • Perform assessments and documentation in accordance with regulatory, ethical, and organizational standards.
  • Maintain confidentiality and ensure all protected health information complies with HIPAA regulations.
  • Team Participation & Professional Development Actively participate in meetings, case reviews, and required trainings.
