About The Position

The Center for Public Health is the only provider of HIV care services federally designated for both Berks and Schuylkill counties in Pennsylvania and serves almost 1,000 people living with HIV/AIDS (PLWHA) annually. As a Ryan White funded clinic, it has a multidisciplinary Ryan White clinical team, including nurses, social worker, nutritionist, behavioral health therapist, chaplain resident, certified registered nurse practitioner, colo-rectal surgeon, family medicine physician, and infectious disease physician. Responsibilities: Plan, coordinate, and oversee special projects that align with Ryan White Program objectives, including service expansion, integration of care, and process improvement initiatives. Develop project plans, timelines, and deliverables in coordination with leadership and key stakeholders. Track project milestones, monitor progress, and provide regular status reports to program management and HRSA partners. Ensure adherence to HRSA/HAB program requirements, reporting timelines, and documentation standards. Support preparation for site visits, audits, and grant renewals by maintaining organized records of project activities and outcomes. Collect, clean, and analyze data from CAREWare, EMR systems, and related databases to support Ryan White Part A/B/C reporting and quality management initiatives. Develop data visualizations, dashboards, and reports to track patient outcomes, service utilization, and program performance. Identify trends and gaps in care and collaborate with program leadership to implement data-informed solutions. Provide analytical support for organizational and program-level strategic planning. Use quantitative and qualitative data to inform decisions on resource allocation, process design, and service delivery models. Serve as a liaison between clinical, administrative, and data management teams to ensure accurate and timely data reporting. Translate complex data findings into clear, actionable insights for leadership and frontline staff. Promote a culture of data-informed decision-making across departments.
