About The Position

Support Nebraskans with developmental disabilities by coordinating their safe and successful transition into community-based services. In this role, you will conduct assessments, develop person-centered service plans, ensure provider compliance, and monitor the quality of supports individuals receive. You’ll work closely with families, service agencies, and the court system to protect individuals’ rights, promote independence, and ensure services meet state and federal standards. This position is key to advancing safe, inclusive, and meaningful community living for the people we serve.

Requirements

  • Bachelor’s degree and professional experience in: education, psychology, social work, sociology, human services, or a related field and experience within a specialized developmental disabilities service system in delivery of habilitation and/or case management services.

  • Knowledge of community-based services for individuals with serious mental illness, person-centered planning, habilitation practices, basic medical terminology, and state and federal regulations.
  • Ability to assess needs, determine eligibility, develop and adjust individualized service plans, monitor service delivery, report non-compliance, communicate effectively with individuals, families, providers, and regulatory agencies, build collaborative relationships, conduct formal assessments, and apply policies while managing program data and priorities.

Responsibilities

  • Coordinate transitions from institutional settings to community-based services in alignment with state and federal requirements.
  • Conduct assessments, determine eligibility, and identify service needs across medical, behavioral, and social domains.
  • Develop and maintain person-centered service plans with individuals, families, and providers.
  • Monitor service delivery through regular site visits to ensure safety, compliance, and quality standards.
  • Investigate complaints, document findings, and support corrective actions with providers.
  • Facilitate planning and transition meetings and coordinate referrals to needed services.
  • Serve as liaison among individuals, families, providers, courts, and state agencies.
  • Prepare required documentation, case plans, and reports; provide testimony in court as needed.
  • Ability to travel required.
