Works directly with parents and caregivers of children ages 0-3 who have identified developmental delays or disabilities; Participates as a member of a multidisciplinary team to perform developmental evaluations and ongoing assessments. Participates as a member of a multidisciplinary team in the ongoing development and execution of Individualized Family Service Plans (IFSP). Works collaboratively with parents and caregivers to develop functional IFSP outcomes. Regularly consults and collaborates with licensed Occupational Therapists, Physical Therapists, Speech Language Pathologists, Registered Nurses, as well as Service Coordinators to provide Early Intervention services according to best practice recommendations, including the ongoing development and implementation of highly individualized, routines-based intervention strategies; Works closely with community members to enhance awareness of early intervention needs and services.
