About The Position

The ideal candidate will work with the Endocrinology and OB teams to support
the care of pregnant patients with diabetes, and assist with tracking,
monitoring, outreach, and supporting all patients involved in this program.

Requirements

• Experience in prior authorization workflow, utilizing various means, including electronically, fax, paper, telephone, and maintaining online spreadsheet, is preferred
• Experience in data collection and tracking, is preferred
• Experience in healthcare settings, particularly in ambulatory or outpatient care, is preferred
• Familiarity with electronic health records (EHR) and spreadsheet software, is preferred
• Excellent communication, interpersonal, and organizational skills, is essential

Responsibilities

• Resource Connection: Serving as a point of contact for patients with diabetes, providing education, coordination, and support for ongoing participation in care.
• Treatment Planning: Supporting individualized treatment plans in collaboration with patients and other healthcare team members.  Assisting patient and clinical teams in obtaining prior authorizations for medication and CGM supplies.
• Advocacy: Advocating for patients' needs and ensuring they receive the necessary support and services. Coordinating CGM initiation, training, and follow-up, ensuring timely device placement and data uploads.
• Documentation: Maintaining accurate and timely documentation of patient interactions, assessments, and interventions. Collecting, organizing, and maintaining patient data related to glucose monitoring, lab results, and pregnancy outcomes in compliance with HIPPA regulations.
• Collaboration: Working closely with other healthcare professionals, such as Physicians, Nurse Practitioners, Diabetes Educators, and Staff to ensure a coordinated and comprehensive approach to patient care.  Supporting quality improvement initiatives and contributing to periodic progress reports as well as participating in team meetings.
