About The Position

RN Director of Case Management Greater Pudget Sound, Washington The RN Director of Case Management drives innovation in patient care and service delivery by fostering collaborative, patient-centered care practices and promoting community engagement. This leadership role oversees a dedicated team of clinical professionals, ensuring seamless Care Coordination across the Continuum. The Director of Case Management works closely with market leaders, physicians, peers, and community stakeholders to achieve organizational goals.

Requirements

• Registered Nurse (RN) license required
• Bachelor’s Degree in Nursing required
• Director of Case Management experience in an Acute Hospital setting

Nice To Haves

• Master Degree in Nursing preferred

Responsibilities

• Leads and evaluates departmental operations, including the Case Management model, staffing (skill mix and FTEs), information technologies, onboarding, and staff competencies to meet performance and quality objectives
• Allocates resources to ensure effective staffing, productivity, and quality goals
• Provides oversight of core Case Management functions and practices
• Ensures adherence to Care Coordination and discharge planning processes, including timeliness, quality, and accurate documentation
• Collaborates with the Facility CFO and Divisional Case Management Leader to achieve established goals and expectations
• Assesses and improves departmental performance by evaluating processes, analyzing data, and implementing sustainable performance improvement activities
• Ensures compliance with policies, procedures, and standard operating procedures (SOPs)
