About The Position

Director of Case Management Greater Orlando, Florida The Director of Case Management leads, manages, and supervises the Case Management Department to ensure seamless Care Coordination and Care Transition handoffs for all patients. The Director provides monthly metric reports and mitigation plans to implement corporate Case Management strategies. The Director meets regularly with the CMO to align Case Management efforts with providers and keeps the CMO informed, while maintaining regular communication with the CFO to ensure bidirectional updates on ongoing Case Management initiatives. The Director of Case Management drives strategy, operational excellence, and alignment across clinical, service, and financial outcomes in a collaborative, matrixed environment.

Requirements

  • Registered Nurse required
  • BSN required
  • Progressive Director of Case Management experience in an Acute Hospital Setting

  • MSN preferred
  • ACM or CCM preferred

Responsibilities

  • Provides monthly metric reports and mitigation plans
  • Collaborates to implement corporate Case Management strategies
  • Meets regularly with the CMO to align Case Management efforts with providers
  • Maintains ongoing communication providing bidirectional updates on Case Management initiatives
  • Implement corporate Case Management strategies and develops proactive approaches to foster a high-performance culture
  • Supports change management initiatives and the shift toward outpatient Care Management integration
  • Collaborate with C-suite and hospital leaders to build strong, professional relationships and achieve shared goals
  • Achieves positive outcomes in complaint investigations
  • While collaborating with other departments
  • Provides leadership and actively participates in identifying, implementing, or enhancing Care Management programs aligned with system strategy
  • Ensures staff practice at the highest level of excellence; oversees recruitment, retention, coaching, mentoring, and leadership development
  • Uses management reports to evaluate budget and staffing variations; supports the Care Management operating budget
