About The Position

The Director, Community and Rural Health Programs will lead the staff, oversight and deliverables related to community health initiatives across multiple states to strengthen dementia care and support through community health engagement, with a particular emphasis on reaching and serving rural communities. This role will work closely with the State Community & Rural Health staff, Mission Engagement Division (MED), Care & Support, Programs and Research, Medical Affairs and other key stakeholders to drive system-level changes by equipping safety-net providers, public health entities, community health centers, and health systems with sustainable, scalable approaches to dementia care and community engagement. This position works to lead the implementation of and adherence to the grant requirements through supervision of engagement managers at the state level. This is a grant funded position.

Requirements

  • Bachelor’s degree required; master’s degree in Community Health, Public Health, Health Administration, or a related field strongly preferred.
  • 7+ years in project and program management over a large footprint such as multi-state or national programs.
  • 5+ years direct experience working with constituents and professionals in community and rural areas such as FQHCs, CHCs, RHCs, or similar safety-net providers.
  • Demonstrated experience in fostering cross-sector collaboration, influencing decision-making, and aligning diverse interests toward shared goals.
  • Grant management experience is required.
  • Willingness to travel up to 15%
  • Demonstrated expertise in strategic planning and governance for multi-state or multi-site initiatives, including setting priorities, managing performance, and ensuring accountability across complex programs.
  • Solid foundation in budgeting, financial oversight, and grant management, with the ability to align spending and resource allocation with contract and grant requirements.
  • Proficient in data analysis, KPI development, and program evaluation methods to measure outcomes, identify trends, and drive continuous improvement. Skilled in translating insights into actionable strategies that enhance impact and scalability.
  • Experience cultivating and sustaining partnerships with FQHCs, Public Health Departments, Rural Health Centers, healthcare systems, and community leaders. Adept at influencing decisionmakers and fostering cross sector collaboration.
  • Deep understanding of culturally responsive practices and equity-focused approaches to reduce disparities in care, particularly in underserved and rural communities.
  • Proven success leading, mentoring, and developing high-performing, geographically dispersed teams. Skilled in creating inclusive, collaborative team environments and coaching leaders toward results.
  • Strong project management skills, including the ability to set clear goals, organize complex work streams, manage multiple priorities, and establish accountability frameworks. Capable of driving coordinated execution across multiple focus areas.
  • Excellent written and verbal communication skills, with the ability to convey complex concepts clearly and build productive relationships with diverse stakeholders.
  • Proficient in Zoom, Microsoft Office Suite, and Google Suite. Comfortable using technology to support documentation, communication, data tracking, and performance management.
  • Skilled in leading large-scale change initiatives, navigating ambiguity, and sequencing implementation to minimize risk and support sustained adoption.
  • Ability to see across markets and programs to design strategies that advance population health outcomes, promote equity, and support long-term organizational sustainability.

Responsibilities

  • Oversight: Provides leadership and comprehensive oversight for Community Rural Health Initiative across multiple focus areas and states. Sets clear priorities, defines success metrics, and establishes accountability structures to ensure initiatives meet program requirements, goals and reporting. Works with state staff to translate national guidance into actionable operational plans to drive effective delivery of all components.
  • Utilization Documentation and Reporting: Defines the strategic framework for utilization tracking, documentation, and performance reporting across all participating states. Establishes processes to secure key performance indicators (KPIs) and accountability measures to assess the effectiveness and impact of Community Health Initiatives. Oversees the development and refinement of systems and processes that ensure accurate, timely, and actionable data for benchmarking and informed decision‑ Provides guidance to internal and external stakeholders on productivity standards and ensures alignment with initiative goals and performance expectations.
  • Strategic Team Leadership: Mentors and develops a geographically dispersed team of engagement managers responsible for advancing community health initiatives. Fosters a collaborative, high‑performance environment by guiding and empowering both internal and external partners. Ensure alignment with organizational goals, state agreements, and long‑term sustainability strategies while cultivating shared ownership for results.
