About The Position

The Director of Nursing (DON) is responsible for the overall direction, supervision, and coordination of clinical services for both the Home Health and Hospice programs. The DON ensures that care is delivered safely, effectively, and in compliance with CMS Conditions of Participation (42 CFR Parts 484 and 418), Utah state regulations, and agency policies as well as joint commission accreditation standards. The DON ensures 24-hour nursing availability, oversees interdisciplinary coordination, leads Quality Assurance and Performance Improvement (QAPI) activities and maintains continuous survey readiness.

Requirements

  • Current, active Registered Nurse (RN) licensure in Utah
  • Bachelor’s degree in Nursing preferred
  • Two (2) years RN experience
  • One (1) year supervisory/management experience
  • Home health and /or hospice experience
  • Current CPR certification
  • Valid driver’s license and reliable transportation
  • Current BLS Certification
  • Knowledge of CMS Home Health and Hospice Conditions of Participation (CoPs)
  • Minimum of 30 words per minute
  • Knowledge of EMR’s
  • Competent in Google Systems (Sheets, Calendar, Document, Slideshows, etc)

  • Bachelor’s degree in Nursing preferred

Responsibilities

  • Direct and supervise all clinical services
  • Ensure care is delivered in accordance with physician orders and individualized plans of care
  • Oversee clinical documentation compliance
  • Ensure patient safety and regulatory adherence
  • Maintain clear lines of clinical authority
  • Ensure appropriate supervision of home health clinical staff
  • Oversee OASIS accuracy and timely submission
  • Ensure home health aide supervisory visits occur per regulation
  • Ensure timely initiation of care
  • Monitor hospitalization and outcome data
  • Ensure nursing services are available 24 hours per day, 7 days a week
  • Oversee symptom management and crisis response processes
  • Ensure RN supervisory visits for hospice aides occur at least every 14 days
  • Participate in and oversee the Interdisciplinary Group (IDG)
  • Ensure comprehensive assessment updates at least every 15 days
  • Lead QAPI programs for both Home Health and Hospice
  • Monitor quality indicators such as: Hospital readmissions Pain and symptom management effectiveness Infection rates Falls
  • Analyze trends and implement corrective actions
  • Prepare reports for governing body review
  • Participate in hiring and orientation of clinical staff
  • Ensure competency validation at hire and annually
  • Conduct performance evaluations
  • Ensure adequate staffing levels for both programs
  • Provide clinical coaching and mentorship
  • Maintain ongoing compliance with CMS and Utah regulations
  • Participate in state and federal surveys
  • Develop and implement plans of correction as needed
  • Ensure policies and procedures are current and implemented
  • Ensure effective 24/7 on-call systems
  • Monitor response times for both programs
  • Oversee documentation of after-hours calls
  • Participate in emergency preparedness planning and annual review
  • Oversee infection control programs for both services
  • Monitor trends and outbreaks
  • Ensure staff training and compliance
