About The Position

Founded in 1866, The Floating Hospital (TFH) is one of the last remaining charity hospitals in New York City. Our commitment is the same as it was 160 years ago: unrestricted healthcare to all who walk through our doors, regardless of identity, insurance status, or the ability to pay. We welcome individuals of all ages, from infants, school-age children (5+), youth and adults. The Floating Hospital is a Federally Qualified Health Center and the largest provider to homeless families living in family shelters and domestic violence safe houses throughout all five boroughs. TFH is seeking an experienced, passionate, and detail-oriented homeless family outreach manager to represent TFH in its relationships with directors of both homeless family shelters and domestic violence safe houses. This managerial position will supervise several outreach staff, will build strong partnerships with case managers and social workers at the non-profits who operate the shelters and safe houses, and ensure effective coordination within TFH departments that serve both populations. Data and reporting experience is expected. Reporting to the Executive Vice President of Clinical Administration/Chief Medical Officer, the successful candidate will oversee up to 5.0 FTEs. This is an on-site, Monday – Friday position, with some evening work related to attending and presenting at workshops/fairs as required.

Requirements

  • Bachelor’s degree in a related field (e.g., Social Work, Public Administration, Communications/Marketing).
  • 3–5 years of experience in healthcare outreach, outreach to vulnerable populations, public speaking, and program management.
  • Proficiency in working with EHR/Microsoft systems and software and exporting/analyzing healthcare data.
  • Excellent communication skills and ability to collaborate with cross-functional teams and all levels of management.
  • Ability to multitask on several projects at once.
  • Federally Qualified Health Center (FQHC) knowledge and experience helpful.
  • Possess a passion and commitment toward TFH’s organizational mission.
  • Valid NYS driver’s license.
  • Strong analytical thinking and problem-solving abilities.

Responsibilities

  • Inform Shelter Directors, Directors of Social Services and Case Managers of the services TFH offers to shelter residents, while also advocating its mission
  • Keep open communication with the Department of Homeless Services
  • Present information to shelter staff about TFH services
  • Conduct presentations to shelter residents about TFH services
  • Enlighten community organizations with presentations to inform them about TFH’s services for the homeless population
  • Create linkage agreements with organizations that run homeless and domestic violence shelters
  • Keep shelter organizations and shelter staff informed about any new initiatives and additional services for the homeless residents
  • Contact organizations that can enhance other services for the homeless population
  • Coordinate presentations, information desks and participate in health fairs for the homeless shelter residents
  • Identify new trends among the families and individuals living in shelters
  • Participate in coalitions that serve homeless families and individuals
  • Liaise with the Department of Education to identify needs of students in temporary housing
  • Ensure health fairs and workshops are scheduled and executed
  • Prepare statistics and reports for upper management
  • Prepare data and participate in Quality Improvement and Management meetings
  • Coordinate collaborations with Community Outreach
  • Assist patients during their visits at TFH
  • Ensure safe and efficient door-to-door transportation to homeless and domestic violence shelter residents.
