About The Position

Our client, h/care Hospice, is looking for a hospice leader who can do both: deliver exceptional clinical care and grow a census in a competitive market. This isn't a role for someone who wants to sit behind a desk. You'll be the face of our Cleveland hospice, building relationships with physicians, hospitals, and families while leading an interdisciplinary team of nurses, social workers, and chaplains. What You'll Do: Own the P&L for our hospice department: census growth, staffing, compliance, margin Build referral relationships with physicians, hospitals, and SNFs Lead and develop an interdisciplinary care team Ensure compassionate, high-quality end-of-life care for patients and families Compete effectively against larger hospital-system hospices What We're Looking For: 5+ years clinical nursing experience, 2+ years in hospice leadership Track record of growing census or building referral relationships Deep knowledge of Medicare hospice regulations and CMS compliance Comfort with EHR systems (WellSky preferred) BSN required, MSN or Healthcare Administration master's preferred Why h/care: h/care was built to be different. We use the Entrepreneurial Operating System (EOS) to cut bureaucracy and move fast. Our PerfectVisit model ensures consistent, high-quality care. We're small enough to be nimble but backed by experienced healthcare entrepreneurs who know how to scale. If you want ownership and impact, not just a title, this is the role.
