About The Position

Kitsap Mental Health Services (KMHS) is seeking a Discharge Coordinator to support clients transitioning from residential treatment at the Pacific Hope and Recovery Center (PHRC). In this role, you will coordinate discharge plans, connect clients with community resources, and help ensure continuity of care following stabilization. This position works closely with multidisciplinary treatment teams to support individuals navigating complex systems and barriers to care. Requirements

  • High School Diploma or GED
  • Entry level (minimum 1 year of job-related experience)
  • Valid Washington Food Handler card.
  • This position requires driving clients in a personal vehicle on behalf of the agency. Incumbent must meet all agency driving requirements, including being at least 21 years old, having been licensed for a minimum of three years, successfully completing a motor vehicle history check, possessing and maintaining a current, valid driver’s license in the state of Washington, and having reliable, insured transportation.
  • Eligibility for Agency Affiliated Counselor Registration
  • Maintain a working knowledge of current trends in community mental health, including health care reform and automation of job tasks using computer technology.
  • Maintain a working knowledge of mental health rehabilitation standards.
  • Excellent interpersonal and verbal skills.
  • Communication - Communicates clearly and concisely.
  • Computer skills - The ability to operate a personal computer, fax machine, printer, and copier proficiently.
  • Ability to deescalate potentially volatile situations with clients.
  • Ability to react quickly and perceive environmental stimuli accurately.
  • Ability to assist in building positive working relationships with staff of all agency departments

Nice To Haves

  • Bachelor’s degree in psychology, social work or a behavioral health field.
  • Experienced (minimum 2 years of job-related experience).
  • Relevant experience working with clients with severe and persistent mental illness.

Responsibilities

  • Coordinate and implement timely discharge plans for clients supported at PHRC.
  • Collaborate with SUDPs, BHCs, and MHPs to develop discharge plans that address complex, multi-system needs and barriers.
  • Support case management tasks related to discharge planning, including coordination of mental health, psychiatric, medical, and substance use services.
  • Assist with housing referrals, transportation supports, DSHS-related services, outpatient medical and dental connections, and follow-up appointments.
  • Participate in daily staff meetings and stabilization planning reviews.
  • Facilitate approved group activities focused on community resources and post-discharge support.
  • Provide transportation to appointments or community resources as needed.
  • Maintain effective communication with unit staff, care teams, and external partners involved in discharge planning.
  • Complete all required electronic and paper documentation in accordance with agency policies and confidentiality standards.
  • In the absence or shortage of Behavioral Health Technicians, assist with unit support tasks such as safety checks, intake and discharge processes, referral coordination, and meal preparation.
