Requirements

• Bachelor’s degree from an accredited college or university in Social Work or related mental health field OR equivalent years of experience
• A minimum of 2 years’ direct clinical experience in a psychiatric or mental health setting, hospital setting is preferred.
• Direct experience in patient/family assessment.
• Experience in working with programs and outside referrals
• A solid understanding of resources in the community.

Responsibilities

• Review and assess new admissions, meeting with patients and families to determine discharge planning needs and collaborate with social workers and therapists to develop treatment plans.
• Attend treatment plan meetings and communicate with patients, families, and outpatient teams to determine placement outcomes and ensure proper discharge planning.
• Coordinate with schools, fax letters to facilitate care coordination, and regularly communicate with outpatient providers to ensure a smooth transition for patients.
• Document progress towards discharge, maintain a discharge calendar, and fax discharge paperwork to ensure timely and effective aftercare placement.
