This position will provide proper record maintenance and appropriate quality care of clients. Will also ensure transition/discharge plan for clients are appropriate and follows rules established by AHCA, SAMH, CARF, Private Insurance and PPBH policy and procedures. Interviewing clients in various settings to obtain information concerning family, emotional, medical, and substance abuse history or other pertinent information. Detail oriented in completing the psychosocial assessments. Discusses the goals and implementation of treatment plans, progress toward goals and develop an appropriate transition plan upon admission. Provide family sessions per protocol. Provide scheduled group therapy and when needed, individual therapy. Responsible for proper placement and appointments required prior to discharge. Formulates appropriate diagnosis, in terms of the current edition of the DSM and ICD, based on pertinent available information. Consults with medical doctors and/or treatment team members concerning treatment goals and objectives. Completes all required paperwork and maintenance of client files according to all applicable regulatory standards. Performs duties as a court liaison. Perform other duties as required by Supervisor.
