Provides an integrated continuum of care for patients from preadmission to post-discharge. Responsibilities Discharge Needs Assessment Consistently performs early assessment and intervenes to ensure positive patient outcomes. Consistently utilizes independent judgment. Documents accurate and concise information in the patient chart to keep all other disciplines informed of plans. Ensures adequate weekly bed availability documentation is performed according to state requirements. Provides counseling as needed. Planning Discharge plans are initiated under the direction of the MSW and utilized according to patient/family needs and desires. Identifies and utilizes available resources. Reassesses and revises discharge plans according to departmental policies and procedures. Implementation Possesses basic knowledge of community and hospital policies and procedures. Implements discharge plans and physician referrals. Updates and completes documentation in accordance with unit standards. Utilizes effective time management. Handles crisis situations as necessary. Transitions patient through the discharge process in a timely manner. Education Possesses basic knowledge of community and hospital policies and procedures. Implements discharge plans and physician referrals. Updates and completes documentation in accordance with unit standards. Utilizes effective time management. Handles crisis situations as necessary. Transitions patient through the discharge process in a timely manner when bed offer is made. Leadership Effectively manages the needs of patients with other disciplines. Assumes a leadership role by displaying a positive attitude with peers, physicians and patients/families.
