- 2 years of college required, Bachelor’s preferred
- M-F 8:30 to 4:30

- hospital related experience is a plus
Responsibilities
- Assist in discharge planning on units as assigned by the Administrative Director of Social Work.
- Work with the treatment team and Program Director on the unit to assist in completion of applications and other discharge plans for the patients.
- Attend morning report and treatment planning.
- Gather necessary documents to complete applications and make appointments, make follow up calls and provide necessary information to outpatient providers.
