The Discharge Planning Coordinator is responsible for designing and coordinating comprehensive discharge plans in accordance with regulatory and patient satisfaction standards. Requirements

  • Bachelor's Degree Accredited School of Social Work
  • 1 year experience with referral coordination

Nice To Haves

  • 2 years experience in a Hospital Clinical setting Preferred
