The Discharge Planning Coordinator is responsible for designing and coordinating comprehensive discharge plans in accordance with regulatory and patient satisfaction standards. Collaborates with nurse case managers and social workers regarding overall discharge planning needs. Your Tomorrow is Here! Requirements
- Bachelor's Degree Accredited School of Social Work
- 1 year experience with referral coordination
Nice To Haves
- 2 years experience in a Hospital Clinical setting Preferred
