About The Position

Under the direction of the Manager of Ambulatory Case Management, the Supervisor, Disease Case Management, is responsible for coordinating and overseeing the delivery of care to high-risk patients enrolled in disease and case management programs. This role ensures that patients with complex medical needs receive comprehensive, continuous care through detailed assessment, follow-up, provider collaboration, and patient education. The Supervisor works closely with frontline disease management staff, clinical providers, and ancillary departments to ensure early identification of eligible patients, appropriate development of care plans, and consistent monitoring of progress. The position also maintains communication across inpatient and outpatient settings to support continuity of care, monitors program effectiveness through data tracking and reporting, and contributes to ongoing quality improvement initiatives. As a working supervisor, this position blends direct clinical case management responsibilities with operational oversight of team workflows, program standards, and quality outcomes.

Requirements

  • Graduate of an accredited Nursing program.
  • 2+ years of case management experience within a managed care or healthcare facility.
  • Current State Registered Nursing License
  • Knowledge of healthcare systems, chronic disease management, and managed care principles.
  • Proficiency in Microsoft Office (Word, Excel, Outlook, Access, PowerPoint).
  • Ability to type a minimum of 40 words per minute with accuracy.
  • Familiarity with standard office and clinical equipment.
  • Strong critical-thinking, communication, and organizational skills.

Nice To Haves

  • Bachelor’s degree in Nursing or related field.
  • Prior supervisory or lead experience in care coordination or clinical operations.
