About The Position

Under the supervision of the Reid Health Transportation Supervisor, performs dispatching and scheduling activities related to the transportation service line. Maintains transportation scheduling software program and conducts scheduling and communication with drivers, Transportation Supervisor, and maintenance personnel. As needed, reroutes trips and driver assignments to meet door-through-door transportation needs of PACE participants. Serves as driver, responsible for transporting and escorting of patients and participants between their homes, Reid Health, Reid Health PACE Center, and other designated locations.

Requirements

  • Minimum Education Required: High School diploma
  • Minimum Experience Required: 1 year in a transportation/patient transport setting/or scheduling environment.
  • Licensure: Valid driver’s license
  • Certifications: CPR
  • Age requirement minimum: 21 years of age

  • Minimum Education Preferred: High School diploma
  • Minimum Experience Preferred: 1 year working with the frail and elderly recommended
  • Age requirement preferred: 25 years of age +

Responsibilities

  • Assumes responsibility for running and maintaining transportation scheduling program.
  • Follows established schedules, adjusts schedules to meet unanticipated changes, and determine the safest and most efficient pick-up and delivery routes.
  • Monitors and ensures effective daily communications, both ways between the drivers, Transportation Supervisor, and the Interdisciplinary Team (IDT) as well as PACE Center staff.
  • Develops, prints, and updates daily work schedules for each driver.
  • Distributes in a timely manner to the Center the following day work schedules for each driver.
  • Coordinates and schedules daily delivery of home delivered meals, Durable Medical Equipment (DME), supplies and needs from the pharmacy, and miscellaneous items as requested by IDT and PACE program.
  • Maintains and updates participants’ database.
  • Assumes responsibility, as needed, for personnel and route substitution(s).
  • Meets with the Transportation Supervisor and interdisciplinary team regularly to discuss participant schedules, bus routes, and other transportation issues to ensure transportation is meeting the needs of the participant and PACE program.
  • Serves as driver when need arises; responsible for transporting and escorting of patients and participants between their homes, Reid Health, Reid Health PACE Center, and other designated locations. Cross trained as transportation escort.
  • Serves as transportation IDT member during morning meeting and care planning, as needed.
  • Follows all PACE Policies and Procedures and OSHA safety guidelines.
  • Maintains the confidentiality of all company procedures, results and information about participants, clients, or families.
  • Participates in continuing education classes and any required staff and training meetings. Maintains professional affiliations and any required certifications.
  • Participates in and is supportive of Quality Improvement initiatives.
  • Performs other duties as required or requested.
  • Compliance with CMS regulations as it pertains to this role.
