About The Position

We are looking for a Document Specialist to join our team at the Ebenezer Corporate Office in Edina, MN! The Document Specialist works both independently and collaboratively to support the Advance Care Planning (ACP) and Surrogate Decision-Maker needs in all regions of the organization. This role validates and scans legal documents identifying Surrogate Decision Makers, updates the medical record in alignment with department and system protocols, and assists the department to provide staff, patients, and community support related to Advance Care Planning and Surrogate Decision-Making. This document specialist schedule includes; 80 hours every two weeks Full time; Day shift No weekends

Requirements

  • Associate of Arts or equivalent combination of education/experience
  • 2 years demonstrated knowledge/experience in: Working both independently and within a collaborative team while achieving goals; Utilizing organizational and communication skills; Building relationships with system teams to achieve specified outcomes
  • 1 year demonstrated knowledge/experience with health-care related legal documents or related experience

Nice To Haves

  • Knowledge of the purpose and process of Advance Care Planning
  • Knowledge of ACP process and resources within health care systems
  • Proficiency in Microsoft applications
  • Proficiency in Epic application

Responsibilities

  • Independently performs validation of legal documents in alignment with department protocols and state statutory requirements
  • Scan legal documents in compliance with department and system protocols
  • Update patient and surrogate-decision maker demographics and authority in compliance with department protocols and state statutes.
  • Assist providers, staff, patients, and other customers with Advance Care Planning and Surrogate Decision Maker requests and inquiries in alignment with department protocols.
  • Collaborate to measure, evaluate and optimize outcomes to meet department and system strategic goals
