About The Position

This position reports to the Director of Supported Housing with the responsibility of supervising the day-to-day operations of the Emerging Adults housing program.

Requirements

  • Bachelor’s Degree from an accredited college or university in social work, psychology or another related field.
  • Three (3) years’ experience in a mental health or social service setting, with at least 2 years working with youth in the same setting.
  • Ability to work independently and with a team.
  • Ability to meet deadlines.
  • Strong verbal and written communication skills.
  • Computer proficient in Word and Excel.
  • Strong professional and ethical behaviors.
  • Valid driver’s license.
  • Must be able to show proof of personal automobile insurance.
  • Must be insurable under Park Center’s automobile insurance.

Responsibilities

  • Maintains all facilities at maximum capacity
  • Receives and reviews all referrals for the housing program.
  • Interviews all prospective participants for the program.
  • Completes intakes on all participants moving into the program.
  • Ensures that houses have needed supplies, groceries, etc. on a weekly basis.
  • Records time on a weekly time sheet, submits to supervisor by deadline.
  • Submits monthly and quarterly reports by deadline.
  • Collects, records, and turns in rent.
  • Performs unannounced quarterly site visits.
  • Develops additional program components as needed.
  • Inspects houses on a weekly basis for cleanliness and safety.
  • Ensures required documentation is completed and filed in a timely manner.
  • Ensures accurate record keeping in participants’ charts.
  • Coordinates cleanliness of houses and grounds.
  • Maintains medication in accordance with licensure standards.
  • Ensures that all other licensure standards are met. cultural competence and diversity
  • Works with services providers and Emerging Adult Program staff to provide appropriate support services.
  • Facilitates house meeting with participants.
  • Develops in house services such as recovery groups.
  • Participates in monthly supervision and weekly staffing.
  • Coordinates transportation for participants in program.
  • Ensure participants have proper medication.
  • Maintains case load of program participants.
  • Develops service plan and transition plan with input from participants within 14 days of admission.
  • Handles crisis situations with face-to-face contact and development of a crisis plan.
  • Transports participants when necessary to appointments and other community activities.
  • Links and refers participants to appropriate services.
  • Assesses participants current and potential strengths, resources, and basic needs in areas of substance abuse, mental status, general health, self-care, support network, training needs and consultation with the family.
  • Utilizes voice and email.
  • Communicates regularly with other staff to meet the needs of participants.
  • Develops appropriate working relationships with other mental health, social services and rehabilitation agencies.
  • Works as a team player with other staff.
  • Assists with development of new housing and program components as needed.
  • Assists with other development duties as assigned.
  • Attends weekly staff and supervision meetings.
  • Assists with overall program events.
  • Participates in on – call rotate on.
  • Has good attendance in the program and informs supervisor in advance of leave or other time off.
  • Follows all personnel policies.
  • Other duties as assigned.
  • Collaborate with staff, members, and the community to gain the perspectives of others having diverse opinions, abilities, values, beliefs, perspectives, and ethnic or cultural backgrounds.
  • Take deliberate steps to increase own cultural competency by attending trainings, events, discussions, workshops, etc.
