About The Position

The ECM Program and Field Supervisor is a frontline program leadership role reporting to the Regional Director of Clinical Operations, responsible for supervising a small team of Community Health Workers while leading the onboarding and development of new staff. This role blends field supervision, patient engagement oversight, and structured CHW training to ensure new hires ramp successfully and ECM workflows are executed with consistency and quality. The position maintains a small patient panel to stay connected to real-time field operations while driving workforce development across Street Medicine and ECM programs. It's a rare opportunity to shape both the people and the practice — building the next generation of field-credible CHWs while directly impacting patient outcomes for some of LA's most vulnerable populations.

Requirements

  • 3–5 years of experience in street medicine, enhanced care management, homelessness outreach, or a comparable high-acuity community health setting (preferred)
  • Strong working knowledge of Medi-Cal managed care requirements, interdisciplinary team structures, and health plan documentation expectations (preferred)
  • Field credible with direct operational experience in outreach environments
  • A skilled educator who can translate complex workflows into clear and practical instruction
  • Knowledgeable of ECM documentation standards and interdisciplinary coordination
  • Organized and metrics-oriented with the ability to maintain structured cadence in a fast-growing environment
  • Committed to protecting team wellbeing and preventing burnout through thoughtful operational design

Responsibilities

  • Supervise 3–5 Community Health Workers, providing day-to-day operational guidance, coaching, and support
  • Conduct regular case reviews to ensure patient engagement plans are appropriate, ECM requirements are met, and interdisciplinary coordination is effective
  • Monitor CHW productivity, encounter cadence, and documentation quality to ensure alignment with program expectations
  • Lead the 0–90 day onboarding ramp for newly hired CHWs, delivering a structured 8–10 week curriculum covering field engagement, ECM workflows, documentation standards, and program expectations
  • Conduct documentation labs and chart review sessions to strengthen encounter quality and compliance with health plan requirements
  • Provide structured field immersion days where new CHWs observe and practice outreach strategy, patient engagement, and partner site coordination
  • Maintain a small patient panel of approximately five patients to remain connected to real-time field operations and model effective outreach techniques
  • Train CHWs on ECM program structure, Medi-Cal expectations, trauma-informed engagement, MAT navigation, domestic violence protocols, and systems navigation for individuals experiencing homelessness
  • Track onboarding ramp metrics including panel growth, encounter activity, and documentation quality; provide structured updates to leadership on staff progression and operational challenges
  • Identify recurring workflow issues and contribute to refinement of training materials, operational playbooks, and program protocols
