The Ochsner Patient-Centered Navigation Program implementing a specially trained Emergency Department (ED) Navigator role to reduce the use of emergency departments for non-emergent reasons. The navigator will achieve streamlined patient care transitions and redirection to appropriate levels of care utilizing hospital and community resources to effectively educate and empower patients and their families.

Requirements

  • Education Required – Associate’s degree with required related experience
  • Work Experience Required - 6 years’ work experience in a clinical setting, case management, home health care or social services setting working directly with patients/clients.
  • Must have computer skills and dexterity required for data entry and retrieval of information.
  • Effective verbal and written communication skills and the ability to present information clearly and professionally.
  • Must be proficient with Windows-style applications, various software packages specific to role and keyboard.
  • Strong interpersonal skills.
  • Strong oral and written communication skills.
  • Proven leadership skills, ability to think critically, problem-solve, work independently, self-initiate, multi-task, and work effectively in a team environment.
  • Highly organized and detail-oriented.
  • Understanding of motivational interviewing techniques.

Nice To Haves

  • Preferred - Bachelor's degree
  • Preferred – Experience in social services or outpatient clinical settings with knowledge of community resources.

Responsibilities

  • Adapts behavior to the specific patient population, including but not limited to: respect for privacy, method of introduction to the patient, adapting explanation of services or procedures to be performed, requesting permissions and communication style.
  • Conduct Initial Evaluation and Engage Patient/Family
  • Identify Patient Needs and Connect Patients to Appropriate Resources
  • Utilize Behavior Change Strategies to Educate Patients and Families
  • Maintain certifications and engage with continuing education opportunities.
  • Other related duties as required.
