About The Position

GENERAL DESCRIPTION: The mission of Metrocare Services is to serve our neighbors with developmental or mental health challenges by helping them find lives that are meaningful and satisfying. We are an agency committed to quality gender-responsive, trauma-informed care to individuals experiencing serious mental illness, development disabilities, and co-occurring disorders. Metrocare programs focus on the issues that matter most in the lives of the children, families and adults we serve. The Enhanced Care Coordinator is responsible for coordinating and/or providing services to individuals with intellectual and developmental disabilities (IDD) residing in an institution (i.e., state supported living center or nursing facility) who are transitioning to a community Medicaid waiver program or community Intermediate Care Facility (ICF/IDD). The Enhanced Care Coordinator also provides assistance in accessing medical, social, educational, and other appropriate services and supports that will help an individual achieve quality of life and community participation acceptable to the individual or Legally Authorized Representative (LAR) on the individuals behalf.

Requirements

  • Bachelor's or advanced degree from an accredited college/university.
  • One year of experience working with Intellectual Developmental Disability Population or Dually Diagnosed (IDD/MH).
  • Basic math skills required.
  • Ability to apply common sense understanding to carry out simple one or two-step instructions.
  • Ability to deal with standardized situations with only occasional or no variables.
  • Use computer, printer, and software programs necessary to the position (i.e., Word, Excel, Outlook, and PowerPoint).
  • Ability to utilize Internet for resources.
  • Current State of Texas Driver License or if you live in another state, must be currently licensed in that state. If licensed in another state, must obtain Texas Driver License within three (3) months of employment.
  • Liability insurance required if employee will operate personal vehicle on Center property or for Center business.
  • Must be insurable by Centers liability carrier if employee operates a Center vehicle or drives personal car on Center business.
  • Must have an acceptable driving record.
  • Additional licensure as appropriate to Degree.

Responsibilities

  • Identifies available community living options, services and supports to assist individuals to successfully live in the community.
  • Coordinates educational activities for service coordinators and other Local Authority (LA) staff about available community services and about strategies to avoid nursing facility placement.
  • Reviews admissions to nursing facilities for individuals with IDD to ensure that community living options, services and supports that could provide an alternative to the nursing facility have been explored.
  • Provides intensive support to ensure an individuals transition to the community to prevent admission or re-admission to an institution.
  • Provides increased coordination and interaction with an institutions interdisciplinary team and the assigned relocation specialist, as appropriate to the individuals needs and situation.
  • Ensures that individuals receive timely assessments of behavioral, medical, nursing, specialized therapies and nutritional management needs, as necessary and as indicated on the individual service plan.
  • Conducts service plan team meetings quarterly, or more frequently if there is a change in an individuals needs or if requested by the individual or LAR.
  • Conducts monthly face-to-face visits with an individual and monitors the delivery of all services and supports while the individual is in an institution and after the individual has transitioned into the community.
  • Performs other duties as assigned.
