About The Position

Our Evaluator directly contributes to our mission of making autism services personal by creating individualized written orders, through collaboration with the client’s full care team, as part of HHF’s Integrated Care Model. This team member will also build relationships with the clients and their local community to bring positive awareness to our business and the ASD community. This is a contract role; we are flexible on hours offered per week and time of availability. Priority will be given to candidates able to work evenings and weekends to meet client needs.

Requirements

  • Passion for making a difference in the lives of our clients.
  • Master’s degree in Professional Counseling or Social Work from an accredited program.
  • Active State appropriate license is required as an LPC or LCSW, & in good standing.
  • 3+ years in the field of Mental Health Counseling (preferably within the pediatric population).
  • Adherence to the LPC and/or LCSW code of ethics and will place the highest value on evidence-based interventions.
  • Evidence of strong written and verbal communication skills with care teams, family members and caregivers of the client.
  • Must pass internal competency within 90 days of hire.

  • Candidate with less experience will be considered if they possess exceptional student experiences with relevant populations and outstanding performance ratings.

Responsibilities

  • In collaboration with the family and other members of the treatment team, designs and directs the recommendation for a child’s treatment by:
  • Reviewing client records (Diagnostic evaluations, medical records, Individualized Education Plans, and/or any other supporting documentation
  • Conduct an interview with the client’s family to discuss the client’s current diagnoses, note symptoms, behaviors and severity, and discuss strengths and areas of improvement in their communication, social, and adaptive living skills
  • Observe the client in their natural environment, noting details on client’s behaviors, communication, social, and adaptive living skills
  • Determine general goals for treatment in the areas of reducing challenging behaviors, and improving communication, social, and adaptive living skills
  • Determine medical necessity recommendations for appropriate treatments, including type of treatment, approximate amount of treatment, and location of treatment
  • Provide proper and timely documentation, including but not limited to
  • Provide written documentation summarizing family interview and observation information, treatment goals and medically necessary treatment recommendations for the client’s written order
  • Session notes documenting services provided
  • Complete re-evaluations annually or as requested by Care Coordination Team
  • Convert sessions as billed or cancelled within the Central Reach platform in a timely manner (within 24 hours of end of session).
