About The Position

United Health Services (UHS) is seeking a board certified Family Medicine Nurse Practitioner (NP) or Physician Assistant (PA) for our UHS Upper Front Street Medical Center- Same Day Clinic Provider position. This is a busy, Family Medicine out-patient practice in a well-established office and we are looking for a Family Medicine provider to handle same day appointments and wellness visits as needed for infants to adult patients. This position is 100% outpatient. Office hours Monday- Friday 8:00 a.m. – 5:00 p.m. with no call.

Requirements

Master of Science in Nursing (MSN) or Physician Assistant degree
Possession of an NPI (National Provider Identification) number, as evidenced by the receipt of a copy of the notification assigning the number
Current NYS Registered Professional Nurse or Physician Assistant License
Documentation of current CPR certification required within one (1) week of hire, with maintenance of current CPR certification during employment
Certified Prescriptive Privilege in NYS
