The Intensive Community Wrap Around Supports program is designed in partnership with the Department of Developmental Services and the Department of Mental Health to create intensive, in-home and community wrap-around services to support adults between ages 18 and 30 with Autism Spectrum Disorder and Serious Mental Illness, or other significant mental health challenges. The goal of this program is to maintain individual stability and effective functionality for people served in their homes and communities by providing clinical services and coaching supports to identify and respond to barriers of obtaining stability and support individual progress while building skills and capacity for everyone. The Family Partner is a peer support role grounded in lived experience. Drawing on personal experience as a parent, guardian, grandparent, or foster parent supporting an individual with ASD and mental health challenges, the Family Partner supports and educates families as they navigate mental health and related service systems. Through shared understanding, they help families access services, advocate for effective treatment, and build collaborative relationships with providers and others involved in care.
