About The Position

The Family Wellness Specialist consults with family case managers and mentors regarding mental and physical health needs of their participants and coordinates connection to healthcare resources. Builds relationships with community partners to connect participants with quality services which are sensitive to the needs of individuals who have experienced homelessness and are living in poverty. Develop wellness coaching relationship with participants to assist them through the process of actively working towards better health by providing support, encouragement, and education.

Requirements

  • Bachelor’s degree from an accredited college or university in a social services related field is required; Masters’ preferred.
  • Knowledge of clinical diagnostic methods and approved modalities of counseling.
  • Knowledge of interviewing methods and coaching techniques.
  • Ability to identify and assess mental health symptoms.
  • Skill in use of crisis intervention techniques.
  • Experience working with trauma.
  • Knowledge of state and local ordinances and policies regarding mental health counseling.
  • Ability to coordinate and/or facilitate trainings and workshops.
  • Ability to communicate effectively, verbally and in writing.
  • Ability to work with a service team or independently as needed.
  • Ability to transport participants in own vehicle as needed.
  • Valid driver’s license with no more than three minor moving violations and/or accidents combined within the last three years, and no major moving violations in the past three years.

Responsibilities

  • Connects with local healthcare agencies to build partnerships which can be utilized by participants to meet their health and wellness needs.
  • Educate family case managers and mentors on health resources available in the community and directly supports case managers and staff as they work to connect their participants with these services.
  • When appropriate, partners with case managers to refer children and adults for psychological evaluations, psychiatric assessments, counseling, mental health skill building, in-home services, parenting programs, etc.
  • Establishes and maintains professional relationships with community mental health providers.
  • Develops, conducts, and/or partners with healthcare vendors to provide on-site health- related wellness training and workshops for families.
  • Reminds participants of appointments and follows up on progress as needed.
  • In accordance with grant requirements, analyze healthcare outcomes of all current participants with a chronic physical health condition by communicating with them at least once per quarter and obtaining relevant health information.
  • Oversees the data collection process of identified needs utilizing monthly report to follow up on outstanding referrals and support staff as needed.
  • Supports participants in creating and implementing wellness goals that include nutrition, exercise, and management of physical health concerns
  • Participates in health and wellness promotion activities (community groups, fairs, trainings, etc.) and regularly contributes to health and wellness campaigns to advocate for solving healthcare gaps for families living in poverty.
  • Coordinate and facilitate on-site health services such as mobile clinics.
  • Coordinate monthly on-site cooking classes.
  • Facilitate on-site community baby showers approximately two times per year. Partner with local agencies such as home visiting programs, mental health agencies, and post-partum services to provide resources and information at the event.
  • Provide on-site task supervision for volunteers seeking clinical experience, such as those studying nursing or medicine.
  • Attends staff meetings and in-service training as needed.
  • Other duties as assigned.
  • Provide service to internal and external customers according to standards as outlined in ForKids Customer CARE Manual.
  • Use agency resources (financial and non-financial) prudently.
  • Acknowledge and follow financial policies of the agency.
  • Accurately complete all required data reporting in accordance with established guidelines.
  • Participate in all scheduled data collection trainings.
  • Participate in development and fundraising activities as needed and requested (ex: meeting with donors, providing tours, attending fundraising events).
  • Regularly engage, support and collaborate with volunteers to provide a meaningful experience.
