As a vital member of our multi-disciplinary Field Team, you will deliver innovative, home- and community-based care that prioritizes the whole person—helping patients achieve better health outcomes, greater independence, and an improved quality of life. Your Role at ConcertoCare: Deliver personalized care to patients in their homes and communities, supporting comprehensive clinical services alongside physician, social work, and behavioral health colleagues. Engage in co-visits, support telehealth appointments, and conduct independent nursing visits to provide holistic, wrap-around support for our patients. Conduct health screenings for new and ongoing patients, ensuring their needs are met from day one and throughout their care journey. Perform essential clinical tasks, including wound care, phlebotomy, medication review, and patient status assessments. Re-engage patients who have become difficult to reach, offering them renewed access to care and support in their home environments. Support your fellow team members by providing cross-coverage during planned and unplanned absences, ensuring no interruption in high-quality patient care. Collaborate with interdisciplinary team members to continuously improve the quality and efficiency of care, directly impacting cost reduction and increasing both patient and staff satisfaction. Contribute to the creation and implementation of clinical treatment plans under the supervision and guidance of our providers and actively participate in team meetings and initiatives.

Requirements

  • Active, unrestricted LPN or RN license in the state of practice (with LPN certification required for LPNs).
  • Current CPR and BLS certifications; IV or blood draw certification or relevant experience.
  • Experience delivering whole-person care, including for patients with complex medical needs, mental illness, and substance use disorders; clinical triage, urgent care, or emergency medicine experience is preferred.
  • Ability and means to travel at least 75% of the time within assigned areas, often to locations with limited public transportation. Previous experience with managed care and/or value-based healthcare delivery is preferred.
  • RNs- Ability to provide clinical supervision for LPNs and/or medical assistants as requested.
  • Proficient computer skills (Microsoft Word, Excel, Outlook) required.
  • DRIVER REQUIREMENTS: Licensed for a minimum of 5 years. No vehicle-related suspensions, DUI, reckless driving, leaving the scene of an accident, vehicular manslaughter, felony convictions, or 3 or more moving violations within the last 5 years. No more than 2 moving violations or 1 at-fault accident in the last 3 years.

  • Experience delivering whole-person care, including for patients with complex medical needs, mental illness, and substance use disorders; clinical triage, urgent care, or emergency medicine experience is preferred.
