Forensic Assertive Community Treatment (ACT) Wellspring is transforming the lives of people with serious mental illness who have been chronically homeless with a team of professionals dedicated to the most effective treatments. First, the FACT team serves people with a history of incarceratioon and mental illness to addresses each individual’s complex needs using a best-practice, person-centered approach to restore skills, rebuild lives and maintain housing. The FACT team wraps each individual with intensive services of a medical team, housing and behavioral health.

Requirements

  • A personal vehicle is required as the FACT TCM will be responsible for traveling between work sites and transporting clients to regular appointments.
  • KY TCM Training
  • BA or BS in field recognized by DBHDID for TCM: psychology, social work or related field and at least one year post Bachelor's working full-time with adults with serious mental illness OR Master's Degree in related field.

Nice To Haves

  • One year experience working with adults with Substance Use Disorders is preferred.
  • The ideal FACT team candidate will have a belief in recovery from mental illness and an enthusiasm for helping persons with disabilities, rooted in empathy and compassion and demonstrating a careful and caring attitude.

Responsibilities

  • Assist clients in gaining access to medical, legal, social, educational and other support services.
  • Develops and facilitates the pursuit of individualized recovery goals via assessment, connecting to resources and monitoring progress.
  • Travels throughout the community conducting home visits and connecting clients with resources and services.
  • Collaborates with landlords and other community partners, advocating for client needs and rights.
  • Assists clients in locating, applying for, and moving into housing.
  • Assists clients with illness management, medication management, and independent living skills, providing crisis intervention as needed.
  • Assisting clients with their engagement in physical, mental health, and addiction treatment, ensuring uninterrupted provision of care.
  • Participates and actively engages in weekly / monthly / quarterly staff meetings, on call rotations, and collaborates daily with Wellspring clinical and program staff as well as other providers and sources of support.
