About The Position

The Habilitation Coordinator manages services for participants in one of the following settings: nursing facility or community. In the nursing facility, the Habilitation Coordinator is responsible for PASRR duties. In community settings, the Habilitation Coordinator is responsible for performing enhanced community coordination (ECC) duties for individuals transitioning or diverting from institutional care. The Habilitation Coordinator is responsible for the following:

Requirements

  • Bachelor’s degree in a social, behavioral, human services field, including but not limited to psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human development, gerontology, educational psychology, education and criminal justice.
  • Texas Driver License (in good standing)
  • One year of experience working directly with individuals with ID or DD

Nice To Haves

  • Strong organizational and computer skills (Microsoft Office and Electronic Health Record System)
  • Knowledge of State Rules relevant to Habilitation Coordination, Service Coordination and PASRR
  • Experience working with individuals with IDD with complex medical and/or behavioral health needs.

Responsibilities

  • Performing required components of habilitation coordinator and/or enhanced community coordination for assigned caseload within the scope of the following programs: Pre-Admission Screening and Resident Review (PASRR) and Home and Community Based Services (HCS) Waiver.
  • Meeting the timeliness requirements in the performance of each required component of habilitation coordinator and/or enhanced community coordination for assigned caseload within the scope of the following programs: Pre-Admission Screening and Resident Review (PASRR) and Home and Community Based Services (HCS) Waiver.
  • Performs the required administrative duties within the scope of the following programs: Pre-Admission Screening and Resident Review (PASRR) and Home and Community Based Services (HCS) Waiver.
  • Adherence to and promotion of the Texana Compliance Policy.
  • Travel within a 6-county region.
  • Work hours: M-F, 8-5 with flexibility based on client need.
