Requirements
Bachelor’s degree preferred, or equivalent combination of education and relevant experience.
Three (3) or more years of experience in one or more of the following:
Community or population-based health screening
Primary care or ambulatory care settings with significant patient interaction
Substance use treatment, intake, or behavioral health environments
Knowledge of substance use disorders, behavioral health screening, motivational interviewing, and harm reduction.
Strong interpersonal and interviewing skills.
Ability to communicate clearly, professionally, and empathetically.
Accurate data collection and documentation skills.
Proficiency with electronic documentation systems.
Cultural competence when working with diverse populations.
Nice To Haves
Familiarity with community-based resources preferred.
Responsibilities
Integrate health promotion, screening, and brief intervention services into Emergency Department patient care.
Identify patients with substance use, behavioral health, or social determinants of health needs and initiate appropriate interventions.
Coordinate referrals and follow-up services with internal teams and community-based providers.
Maintain accurate, timely, and confidential documentation supporting Project ASSERT outcomes.
Collaborate with Emergency Department clinicians and support staff to support patient-centered care.
