Requirements

  • Bachelor’s degree preferred, or equivalent combination of education and relevant experience.
  • Three (3) or more years of experience in one or more of the following: Community or population-based health screening Primary care or ambulatory care settings with significant patient interaction Substance use treatment, intake, or behavioral health environments
  • Knowledge of substance use disorders, behavioral health screening, motivational interviewing, and harm reduction.
  • Strong interpersonal and interviewing skills.
  • Ability to communicate clearly, professionally, and empathetically.
  • Accurate data collection and documentation skills.
  • Proficiency with electronic documentation systems.
  • Cultural competence when working with diverse populations.

Nice To Haves

  • Familiarity with community-based resources preferred.

Responsibilities

  • Integrate health promotion, screening, and brief intervention services into Emergency Department patient care.
  • Identify patients with substance use, behavioral health, or social determinants of health needs and initiate appropriate interventions.
  • Coordinate referrals and follow-up services with internal teams and community-based providers.
  • Maintain accurate, timely, and confidential documentation supporting Project ASSERT outcomes.
  • Collaborate with Emergency Department clinicians and support staff to support patient-centered care.
