About The Position

Join our compassionate hospice team as a PRN (as-needed) Home Health Aide (HHA), delivering vital care to patients in their homes and community settings. This dynamic role offers flexible scheduling and the opportunity to make a meaningful difference in the lives of individuals facing serious illnesses. As an HHA Hospice PRN, you will provide personalized, respectful care that promotes comfort, dignity, and independence for patients during their most vulnerable moments. Your dedication will support families and caregivers, ensuring they feel supported and informed every step of the way. Perform patient observation and monitor vital signs regularly to assess health status and detect changes promptly. Assist with Activities of Daily Living (ADLs), including bathing, grooming, dressing, toileting, and mobility support using tools like Hoyer lifts. Administer medications and manage medication documentation accurately in compliance with HIPAA regulations. Support patients with dementia, Alzheimer's, memory care, and other cognitive impairments through specialized care techniques. Utilize Electronic Health Record (EHR) systems to document patient interactions, medical records, and care plans efficiently. Provide hospice-specific care such as catheter care, wound management, and symptom management to promote comfort. Assist with light housekeeping tasks including cleaning, laundry, meal preparation, and ensuring a safe environment for patients. Work collaboratively with nursing staff and other healthcare professionals to ensure comprehensive patient care in home and community settings. Follow all safety protocols including CPR certification and proper use of medical equipment to maintain a safe environment for patients and staff. Maintain strict confidentiality of patient information in accordance with HIPAA standards while providing compassionate home & community care. Prior experience working in assisted living facilities, nursing homes, or home health environments is highly preferred. Demonstrated ability to work effectively with people with disabilities or developmental disabilities. Experience caring for patients with dementia or Alzheimer’s disease is desirable. Knowledge of medical terminology, medication administration, catheter care, and hospice protocols is essential. Proven ability to handle medical documentation accurately within EMR/EHR systems. Long-term care experience or live-in employment background is advantageous. Strong caregiving skills combined with excellent communication abilities to support patients’ physical and emotional needs. Requirements
