About The Position

High Needs Case Manager: JFCS is seeking a compassionate professional to work with children receiving behavioral-health services as they progress on the path to a more self-sufficient lifestyle and potential family reunification. The High Needs Case Manager also provides guidance and ongoing support to the families/caregivers of the youth receiving services.

Requirements

  • Must meet one of the following education/experience requirements: High School Diploma/GED required and 4 years of Behavioral Health experience A.A. Degree and two years of Behavioral Health experience Bachelor’s Degree in Social Work, Counseling, Marriage and Family Therapy, Psychology or related field.
  • Additional qualifications: Level 1 Fingerprint Clearance Card without Restriction(s) Meet Agency requirements for MVR (Motor Vehicle Record) report Reliable transportation AZ driver’s license AZ auto insurance

Responsibilities

  • Provides High Needs Case Management services to children enrolled in the behavioral-health system. Included in the scope of services is weekly contact with clients, home/community visits, attending medication checks, etc.
  • Assists youth in navigating discharge from hospitals, in finding intensive services as clinically-indicated, and working to decrease behaviors to improve stability in the home and community.
  • Completes referrals, prior authorizations, and other paperwork required by the health plans to enroll clients in services that are clinically-indicated.
  • Responsible for coordinating and facilitating Child and Family Team (CFT) Meetings.
  • Collaborates with clinical team to ensure that Integrated Service Plans, Child and Family Team (CFT) Meetings, CASII/ECSII, and other required paperwork is completed accurately and remains current.
  • Collaborates, and coordinates services, with other involved agencies, such as Department of Child Safety (DCS), Probation/Parole Departments, Division of Developmental Disabilities (DDD), schools, and other providers.
  • Works effectively within the Arizona Recovery Model as outlined by the 12 principles, including: Accessible services, Best practices, Collaboration with families, Collaboration with others, Connection to natural supports, Functional outcomes, Independence, Most appropriate setting, Respect for the child and/or family's unique cultural heritage, Services tailored to the family, Stability, and Timeliness.
