HIV PrEP Navigator

About The Position

The PrEP (Pre-exposure Prophlaxis) Navigator will provide patient support and navigation services to HIV negative individuals who are at higher risk for becoming infected by HIV and/or who may have barriers to accessing and adhering to PrEP care. In addition, this individual will support the development of systems and infrastructure to increase PrEP services within Katy Trail Community Health and other local community systems. These services will be provided either in-person or via phone at a Katy Trail Community Health clinic, and at off-site local community locations. The PrEP Navigator has to be able to work with a team, be self-motivated and operate in a fast-moving environment. Must be able to demonstrate cultural humility as well as an understanding, awareness, and respect for diversity among colleagues and clients. The following is not an all-inclusive list of tasks. Other duties may be assigned.

Requirements

High school diploma or GED equivalent
One year of work and/or volunteer experience working in HIV prevention or care, medical care systems, diverse populations, and/or underserved communities
Basic understanding on how HIV is transmitted and prevented
Experience with consistent disproportionately affected populations
Demonstrated ability to related to diverse populations
Strong verbal and written communication skills
Strong interpersonal and organizational abilities
Must be able to travel
Must have a valid driver's license with appropriately insured personal vehicle for work-related activities
Must be able to work evenings and weekends
Demonstrate good work habits, such as attendance, punctuality, accountability, thoroughness, and resourcefulness. Previous experience in the operation of office machinery: personal computer, copier, FAX, postage machine, printers.
Adequate vision. Ability to reach, stoops, walk, and lift 25 pounds without device assistance
BLS Certification or/and the willingness to acquire through KTCH
