About The Position
Join Loyola Medicine Home Care and Hospice Join Loyola Medicine Home Care and Hospice, a member of Trinity Health At Home, as a Master Social Worker (MSW). In this meaningful role, you’ll provide compassionate, one-on-one care to patients in their homes, helping them navigate emotional, psychological, and safety challenges during their care journey.
Requirements
- Master’s Degree in Social Work (MSW)
- Licensed Social Worker in the State of Illinois
- Minimum 1 year of experience in a healthcare setting (home care/hospice preferred)

Responsibilities
- Conduct initial assessments to identify patient needs
- Develop and implement individualized care plans in collaboration with physicians and interdisciplinary teams
- Connect patients and families with appropriate community resources
- Ensure compliance with federal, state, and local regulations
- Participate in quality improvement initiatives
