Home Care Service Coordinator
About The Position
Position Summary: Responsible for scheduling and supervising in-home care workers and clients in a geographic area. Supervisory and/or home care experience preferred. Hours: Full Time: Monday through Friday 8 am to 5 pm. Plus rotating on call every other week taking afterhours calls; travel within local geographical territory. Requirements
- Must have high school diploma or equivalent
- 1 year of scheduling experience is preferred
- 1 year home care experience
- Interpersonal, organizational and communication skills
- Computer skills including but not limited to Microsoft Word, Microsoft Excel, use of multiple systems
- Must have reliable transportation

Responsibilities
- Schedules employees as directed by client's care plan established upon intake.
- Creates work schedules by entering schedules into the system, manages changes to client schedules due to client request, illness, vacation or leaves of absence.
- Provides alternate coverage to ensure the client's care plan is followed and client services are not interrupted.
- Complete client intake paperwork with routine follow up visits.
- Monitor systems to ensure staff are clocking in and out for scheduled shifts.
- Supervises direct service employees by setting expectations for attendance, performance and conduct by holding employees accountable to the company's policies and guidelines.
- Cross train for new hire process of new employees and ensures all documentation is completed accurately and in a timely manner.
