About The Position

The Home Health Aide/Homemaker Float plays a critical role in providing compassionate, personalized care to individuals in their homes, ensuring their comfort, safety, and well-being. This position involves delivering a combination of personal care services and homemaking tasks to support clients who may have physical, cognitive, or emotional challenges. The float aspect of the role requires flexibility and adaptability, as the aide will serve multiple clients across different locations, responding to varying schedules and care needs. By fostering a supportive and respectful environment, the aide helps clients maintain their independence and quality of life within their own homes. Ultimately, this role contributes significantly to the health care continuum by enabling clients to remain in a familiar setting while receiving essential care and assistance.

Requirements

  • High school diploma or equivalent.
  • Completion of a state-approved Home Health Aide training program.
  • Certification as a Home Health Aide (HHA) or equivalent, as required by state regulations.
  • Valid driver’s license and reliable transportation to travel between client locations.
  • Ability to pass background checks and meet health screening requirements.

Nice To Haves

  • Previous experience working as a Home Health Aide or in a similar caregiving role.
  • CPR and First Aid certification.
  • Familiarity with electronic health record (EHR) systems or mobile care documentation tools.
  • Strong knowledge of community resources and social services available to clients.
  • Bilingual abilities to communicate effectively with diverse client populations.

Responsibilities

  • Provide personal care services such as bathing, grooming, dressing, toileting, and mobility assistance tailored to each client’s needs.
  • Perform homemaking duties including meal preparation, light housekeeping, laundry, and shopping to maintain a safe and clean living environment.
  • Monitor clients’ health status by observing physical and emotional conditions and reporting any changes to the supervising nurse or care manager.
  • Assist with medication reminders and ensure clients adhere to prescribed care plans under supervision.
  • Travel between client homes as assigned, managing time efficiently to meet scheduled visits and provide consistent, high-quality care.
  • Float between all 3 service areas as needed
