The Home Health Aide PRN plays a vital role in providing compassionate, personalized care to patients in their homes across the United States. This position focuses on supporting individuals with daily living activities, ensuring their comfort, safety, and well-being while promoting independence. The aide will work on an as-needed basis, providing flexible support tailored to each patient's unique needs. By closely monitoring patients' conditions and reporting changes to healthcare professionals, the aide contributes to effective care planning and timely interventions. Ultimately, this role enhances patients' quality of life by delivering attentive, respectful, and professional home health services.

Requirements

• Current CNA/HHA & LTC certifications
• Valid Driver’s License
• Valid Auto Insurance
• CPR Certification

Nice To Haves

• HomeCare HomeBase experience preferred, not required

Responsibilities

• Provide direct patient care to patients under direction of the RN and according to the Aide Plan of Care and to include personal hygiene, ambulation, exercise, safe transfers and other supportive tasks as assigned.
• Appropriately report changes in the patient’s condition.
• Complete appropriate documentation in a timely manner.
• Practice accepted infection control principles.
• Promote positive, supportive, respectful communication to patient/family and other employees.
• Provide a clean, safe and comfortable environment which promotes respect for patient, privacy and property.
• Provide skills necessary to assist the patient with proper nutrition and adequate fluid intake.
• Promote the agency philosophy and administrative policies to ensure quality of care.
