About The Position

Midland Care Connection Midland Care Connection is a not-for-profit, community-based organization dedicated to helping older adults and seriously ill individuals stay independent and supported in their homes. Established in 1978, we offer a full continuum of care, including hospice, palliative care, home health, grief and loss counseling, and our signature PACE (Program of All-Inclusive Care for the Elderly) program. Our mission centers on "meeting individual needs through innovative care solutions," and we serve people across multiple counties in northeast Kansas. Community Care Hospice One of Midland Care Connection’s hospice programs, offered through its Community Care Hospice partnership, provides compassionate, personalized end of life care to individuals and their families in central and south central Kansas. As a not for profit organization with decades of experience, Midland Care focuses on comfort, dignity, and support for those facing life limiting illnesses, delivering services tailored to each person’s needs at home, or in partner facilities. Through the Community Care Hospice joint venture with local non profit retirement and senior living communities, the program extends this expertise across multiple counties, combining skilled clinical care with emotional and spiritual support to help patients and families navigate the final stages of life with compassion and respect. Home Health Aide Provides hands-on personal care and comfort support to patients receiving end-of-life care at home. Working under the supervision of a registered nurse, the aide assists with bathing, grooming, dressing, toileting, turning, and repositioning to maintain comfort and prevent skin issues. They may also help with meals, feeding, light housekeeping, laundry, and changing bed linens. In addition, aides observe the patient’s condition and report any changes to the hospice nurse. Their role focuses on maintaining dignity, comfort, and quality of life while also giving family caregivers practical support and relief. Hours Monday - Friday | 8:00am - 5:00pm Pay Rate $20.15+/hour Bonus $1,500 To be paid in two increments at 6 months and 1 year of employment

Requirements

  • Current Home Health Aide certification
  • Ability to provide companionship and assist with daily living activities (e.g., mobility, meal support, safety) in clients’ homes
  • Help clients stay safe, comfortable, and connected in their own homes
  • Travel within the community to deliver care and support

Nice To Haves

  • At least one year of experience working as a HHA
  • Prior experience in home health, hospice, long-term care, or assisted living setting
  • CPR and First Aid certification
  • Strong communication and interpersonal skills

Responsibilities
