Requirements

  • Active Florida RN license and current CPR certification
  • A minimum of 2 years of experience within a home health agency
  • Strong expertise in OASIS, Medicare regulations, and Conditions of Participation
  • 1–2+ years of clinical leadership experience with the ability to confidently mentor, coach, and hold clinicians accountable
  • EMR proficiency in a home health setting
  • Proactive, confident leader who thrives in a fast-paced Medicare environment and is passionate about elevating patient outcomes, strengthening quality metrics, and supporting high-performing clinical teams

Responsibilities

  • Provide clinical oversight for patient care services, including assigning staff, coordinating patient care, managing referrals, and ensuring patient needs are continuously assessed and addressed
  • Support clinical documentation quality, including OASIS accuracy, Medicare compliance, and adherence to Conditions of Participation
  • Collaborate with the interdisciplinary healthcare team to ensure coordinated care delivery and effective discharge planning
  • Ensure ongoing interdisciplinary assessment of patients while supporting individualized plans of care that reflect patient goals and physician orders
  • Mentor and guide clinicians to strengthen clinical decision-making, documentation practices, and patient outcomes
  • Participate in the Quality Assessment and Performance Improvement (QAPI) program, identifying opportunities to enhance quality, compliance, and operational efficiency
  • Support survey readiness and ensure care delivery aligns with state and federal home health regulations

Benefits

  • Salary: $85,000 - $90,000
  • Full-time, Monday through Friday in the office 8:30 AM-5:30 PM
