Requirements
- Degree from an APTA-accredited physical therapy program
- Active state license to practice as a Physical Therapist
- At least 1 year of experience or completion of a 6–8 week preceptorship
- Strong organizational, communication, and tech skills
- Valid driver’s license and reliable transportation
Nice To Haves
- Home health experience and knowledge of Medicare Conditions of Participation preferred
Responsibilities
- Deliver high-quality physical therapy services in a home setting
- Conduct assessments and develop individualized care plans
- Educate patients and caregivers on exercises and mobility techniques
- Collaborate with interdisciplinary teams to ensure holistic care
- Use advanced, user-friendly technology to document and track progress
