About The Position

The Home Health RN Case Manager provides skilled, patient-centered nursing care to individuals in their homes, promoting independence, safety, and optimal health outcomes. This role is responsible for managing an assigned caseload, coordinating interdisciplinary services, and ensuring each patient's individualized plan of care is developed, implemented, and evaluated in accordance with VNA policies, clinical best practices, and applicable state and federal regulations.

Requirements

  • Current Registered Nurse (RN) license in the State of Kansas.
  • Minimum of one (1) year of nursing experience; home health experience preferred.
  • Current CPR certification.
  • Valid driver's license, reliable transportation, and current auto insurance.
  • Strong assessment, critical thinking, organizational, and communication skills.
  • Proficiency with electronic health records and clinical documentation systems.

Nice To Haves

  • Experience with wound care, chronic disease management, and patient education preferred.

Responsibilities

  • Conduct comprehensive nursing assessments and develop individualized home health plans of care in collaboration with the interdisciplinary team (IDT).
  • Provide skilled nursing services including medication management, wound care, disease management, patient education, and clinical interventions as ordered by the physician.
  • Coordinate care with physicians, therapists, social workers, home health aides, and other healthcare professionals to ensure continuity and quality of care.
  • Monitor patient conditions, identify changes in status, and promptly update care plans as needed.
  • Educate patients and caregivers on disease processes, medications, treatments, and safety in the home environment.
  • Maintain accurate, timely, and compliant clinical documentation in the electronic health record (EHR) in accordance with Medicare and agency standards.
  • Complete OASIS assessments accurately and within required timeframes (training provided if needed).
  • Participate in IDT meetings, case conferences, and assigned on-call rotation.
  • Ensure compliance with infection control, safety, confidentiality, and quality assurance standards.
  • Provide guidance and oversight to Licensed Practical Nurses (LPNs), Home Health Aides, and other field staff as applicable.
